Requesting Proof of Coverage (For Life Events)

Are you adding or removing a spouse or dependent on your Snowflake benefits? Often, current or previous insurance providers or employers will require formal documentation before they can be “released/added” from their old plan.

If you need a letter to prove that you or your dependents are gaining/losing coverage at Snowflake, please follow the steps below.

What Documentation Can I Get?

Our Benefits Advocates can provide a formal Enrollment Verification Letter on employer letterhead. This letter typically includes:

  • The names of all individuals gaining coverage.

  • The specific benefit plans they are enrolled in.

  • The Effective Date (eligibility date) of the coverage.

How to Request Your Letter

To ensure your letter is accurate and generated quickly, please email our Benefits Advocacy Team:

  • Email: snowflakebenefits@alliant.com

  • Subject Line: Enrollment Verification Request – [Your Full Name]

  • Information to Include: * Full names of the dependents being added/deleted.

    • The reason for the request (e.g., “Moving children from spouse’s plan to/off Snowflake”).

    • Any specific details requested by the other employer (such as a request for specific letterhead).

Additional Resources

In many cases, a copy of your Insurance ID Card or your Open Enrollment Confirmation Statement may also serve as proof of coverage. You can typically download these directly from the carrier’s member portal (e.g., Cigna, Delta Dental, etc.).
