This is a voluntary benefit that you enroll in either as a new hire or during Open Enrollment. You are eligible for this benefit if you are a regular or intern employee scheduled to work 20+ hours per week.
You pay the cost of coverage: $4.61 per pay period for just yourself or $8.76 for yourself plus dependents.
Please note, that if you wish to cover your dependents under this benefit, they must have a valid Social Security Number on their dependent record in Empyrean.
