What Is Federal COBRA?

The Consolidated Omnibus Budget Reconciliation Act (COBRA) allows you and your covered family members to temporarily extend health coverage—including Medical, Dental, Vision, Lyra Mental Health, and Healthcare FSA—when active benefits end.

Life Stages that Qualify for COBRA

COBRA is not only for employees who leave Snowflake. You qualify under any of the following life events:

  • Leaving Snowflake: Voluntary or involuntary job departure (for reasons other than gross misconduct).
  • Hours Reduction & Extended Leave: Moving to part-time status or taking extended unpaid leave that results in loss of benefit eligibility.
  • Dependents Life Stage Change: A covered child reaching the age limit of 26.
  • Divorce or Legal Separation: Loss of spouse or dependent eligibility due to marital status changes.
  • Employee Medicare Entitlement or Death: Protecting dependents when the primary covered employee transitions to Medicare or passes away.

How to Enroll in COBRA

If you become ineligible for Snowflake’s health and welfare benefits, we notify the carrier of your qualifying event on a weekly basis. A benefits packet will be sent to you via U.S. mail within 14 days of your termination date by the COBRA administrator, Empyrean, for your options under COBRA.  If you decide to enroll in COBRA, you may either return the enrollment paperwork sent by Empyrean or call Empyrean at (833) 874-1600 Monday through Friday, from 7 a.m. to 7 p.m. CT to enroll.

When COBRA coverage is elected and payment is received, coverage is retroactive back to the first day of loss of coverage. You and your dependents are required to pay 102% of the cost of the premium which includes an administrative fee. View our current COBRA premiums.
